Prakash O. Shah, 64, of Rockford passed away Monday, June 25, 2018. Born April 11, 1954 in India to Ochhavalal and Bhanuben (Talati) Sheth. Married Preeti Parikh.

Survivors include wife, Preeti Shah; children, Puja (Neel) Jani, Pathik Shah; 2 brothers; 2 sisters; and father, Ochhavala. Predeceased by mother.

Funeral service will be held at 3:00 p.m. Thursday, June 28, 2018 in Honquest Family Funeral Home with Crematory – Mulford Chapel, 4311 N. Mulford Road, Loves Park.
